BP at Home

Cambs GP Network are pleased to be able to offer support to GP practices and their patients who may require a home blood pressure monitor to self-monitor their blood pressure.

 

The aim of the service is to help to identify those patients who may have undiagnosed hypertension, or who have known hypertension but may need their treatment adjusting.

 

Patient referral to CGPN is via the patients registered GP Practice, a CGPN Health Care Assistant will arrange a time to visit the patient to deliver a BP machine and demonstrate how to use it, the patient will  complete 7 days of BP readings which will be collated by CGPN and forwarded to the patients registered GP Practice ensuring any escalations are raised in a timely manner.

 

CGPN also offers support to the patient whilst they are self-monitoring with a check in call.

 

The service is available to all patients who are registered with a GP Practice within the CGPN Footprint.
