Cherry Hinton Medical Centre / Brookfields Health Centre, Cambridge.

​

We are seeking an experienced and enthusiastic Primary Care Nurse with a strong understanding of quality assurance.

The postholder will actively engage in practice / patient engagement and will play a vital role in providing high-quality care to our patients.

​

Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management, and preventative nursing interventions to all patients.

​

Assist the practice and its staff with compliance of infection prevention and control policies and procedures

​

Assure clinical safety and drive excellent patient outcomes

​

Support the practice manager and clinical lead in service delivery

​

Work collaboratively with the whole practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Main Duties of the Role

​

  • Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.

  • To identify, manage and support Patients at risk of developing long term conditions, preventing adverse effects to patients health

  • Conduct regular reviews and assessments of patients with long-term conditions, adjusting treatment plans and providing education to optimise self-management.

  • Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services, as necessary.

  • Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.

  • Administer vaccinations and immunisations, including travel (depending on individual competency), according to national guidelines and practice protocols.

  • Support and manage health needs of women presenting with contraception and cervical screening.

  • Complex wound management

  • Venepuncture and request Pathology services as required

  • Support the team in dealing with acute clinical requests and clinical emergencies

  • To act as chaperone for patients as needed

  • To support the clinical team with all safeguarding matters in accordance with local and national policies

  • The role of the Practice Nurse is an evolving one and it is anticipated that the post holder will be responsible and reasonably flexible in the interpretation of the above job description.

Shifts are weekdays 1830hrs to 2200hrs and weekends 0900-1700hrs.
